  • Infection Control Risk Assessment, Model Template

    This document is a template for a comprehensive Infection Control Risk Assessment. This tool addresses the key elements of the Joint Commission's infection control standard related to required components of an infection control risk assessment. The assessment is built around an assessment of events, probability of occurrences, patient effect, intensity of the organization's response needed to address the risk, and the organization's preparedness to address the risk at the current time; with these elements scored in conjunction with important factors such as IC issues related to geography and community, resistant organisms, services provided, staff issues and communication, environment, supplies and emergency management. The assessment drives a score for the important components and allows the organization to prioritize based on the severity of the total score.

  • International Health Reporting Guidelines

    On June 15, 2007, the revised International Health Regulations went into effect, with the purpose of helping protect the world from a host of emerging diseases and health threats such as pandemic influenza and bioterrorism. Infection Control Practitioners in our hospitals are familiar with the requirements of reporting specified diseases to their local health departments. The implementation of the updated International Health Regulations should be seamless, with notification forwarded by the local authorities to the State Department of Health and then forwarded on to international authorities. Although there is no international "hands on" involvement at the hospital level, it is important to have knowledge of the process in planning for pandemic flu response. This document is a discussion of the reporting requirements.
