Pain assessment and reassessments are among the most frequently cited standards, especially the documentation of reassessments after intervention (PC.01.02.07 EP.3). The lack of a comprehensive initial pain assessment is also frequently cited (PC.01.02.07 EP.1), but hospitals can typically show 90% compliance for emergency department patients and inpatients. Compliance is less clear for the general ambulatory patient.The following model policy addresses common vulnerabilities by establishing common sense approaches to this difficult issue. It narrows the patient populations requiring a comprehensive assessment and it allows for flexible documentation of pain reassessments.The policy calls for 12-hour pain reassessments for inpatients to cover hospitals with a mix of 8 and 12 hour shifts. By doing reassessments on each shift both 8-hour and 12-hour nurses will comply with policy without raising single standard of care issues.
